Enable stewarding & incident reports

Adds a “Report an Incident” button on the Rules page and lets members file reports.

Overall stewarding channel (optional)

Used for every incident unless a series below has its own channel. Leave as “None” to keep decisions private (DM only) by default.

Per-series channels (optional — overrides the overall channel for that series)

An incident posts to its series’ channel if one is set; otherwise it falls back to the overall channel above. General incidents (no race/series) always use the overall channel.
